Role Overview:
Our client is seeking an experienced Nurse with a passion for delivering high-quality Stoma care services within the community. The ideal candidate will possess relevant clinical skills, along with the drive and commitment to excel in this role. As a Stoma Care Nurse, you will work as an independent practitioner, demonstrating self-motivation and a strategic mindset. You will be instrumental in developing and delivering their Stoma care services, while also enhancing your commercial awareness and collaborative skills.

Responsibilities:

• Provide professional and ethical care to patients requiring Stoma care services.
• Support patients transition from hospital to home by conducting home visits and community-based clinics.
• Conduct virtual consultations via telephone and video platforms.
• Efficiently plan, manage, and organise caseload and time to ensure optimal patient care.
• Communicate effectively with patients, caregivers, and referring healthcare professionals, providing feedback as necessary.
• Maintain accurate clinical records in compliance with company, legal, and professional standards.
• Establish working practices to facilitate collaboration between different stakeholders, including acute and community healthcare teams.
• Foster and maintain strong working relationships with healthcare professionals across the region.
• Deliver educational support to fellow healthcare professionals.
• Participate in service audit and evaluation activities to ensure continuous improvement.

Essential Requirements:

• Registered General Nurse.
• Bachelor of Science in Nursing degree or working towards completion.
• Stoma care experience is desirable but not essential.
• Strong interpersonal and relationship-building skills.
• Proficiency in IT.
