AssessPatients is an eHealth start-up that was established in 2013 by Richie Bowden, a veteran of the IT and Software Solutions Delivery industry with over 25 years of experience in public and private healthcare organisations. We are developing a web-based platform to simplify the assessment of care needs and risks for health & social care organisations offering Older Care services, such as Acute Care Hospitals, Residential & Community-based Health & Social Care Programmes. We believe in a user-centered approach, which involves close collaboration with clinicians, carers, social workers and therapists to make sure that the decision support solution is tailored to the relevant problems and made easy to use through software. Hospital & Nursing Home in Dublin to test our software in the admission process and older care pathway. Additionally, we are in talks with several UK-based healthcare organisations to agree on live pilots to prove the business case for better quality of care and cost savings. AssessPatients’ features can help multi-disciplinary care teams to enhance patient care through better analysis and preventive measures, such as: an online library of recognised multi-disciplinary assessments for Physio & Occupational therapy, Social Worker and Nursing/Carer practices (e.g. Falls Risk, Cognitive Ability, Mobility); the ability to manage and track health & social patient outcomes related to quality of life; online alerts to the appropriate members of the multi-disciplinary team based
